Download remote monitoring resources for primary care clinicians
Over a six month period, five North East practices piloted the use of existing remote monitoring technology to develop use cases for clinical pathways that would have the biggest impact on patient outcomes.
On the Health Innovation North East and North Cumbria (HI NENC) website, primary care teams can now find tried and tested resources that demonstrate how technology already at their disposal can be used to help support their capacity and demand needs whilst also providing better patient outcomes.
HI NENC also hosted a webinar to present the clinical pathways to primary care teams. The session, led by expert clinicians, shared best practices and case studies. The webinar is ideal for staff in primary care networks (PCNs), including nurse practitioners, pharmacists, and GPs.

In addition to the above resources, HI NENC has a number of digital opportunities for primary care colleagues, including free training through the Digital Champions programme. The course is a CPD-accredited 12 month education programme, with a value of approximately £700 in free training.
