Remote monitoring pathway resources – for general practice

The below tried and tested resources have been developed for general practice staff who would like to embed new remote monitoring clinical pathways within their organisation. Resources include standard operating procedures for several pathways including: blood pressure monitoring, UTI, oral contraception, and more.

About the programme

In 2023 HI NENC was commissioned by the North East and North Cumbria Integrated Care Board (NENC ICB) to deliver a pilot to explore and expand the use of remote monitoring in general practice. With demand for services increasing, the project was set up to find new ways to deliver care safely and effectively, and free up clinicians in primary care from administrative processes.

As part of this project, five North East practices used existing remote monitoring technology over a six-month period to develop use cases for clinical pathways that would have the biggest impact on patient outcomes.

The programme has now developed a catalogue of solutions so that these innovations can be embedded within other practices. Materials, such as standard operating procedures and flow charts, have been created so that these pathways can be adopted across the region. Please find these resources below.

Following a successful pilot, HI NENC is seeking expressions of interest for practices who would like to be involved in a second wave of the programme, involving 20 practices. Contact Anne Richardson by 9th August 2024 to express your interest.  Please note, to be involved in the programme, you will need to commit to a full day session on 12th September or 16th October 2024 in Newcastle City Centre.

Resources

The resources below include the first developed standard operating procedures (SOPs) covering a number of areas in general practice. The SOPs provide a step by step guide along with a flow chart for practices to follow to implement in their own practice. You will also find supporting short videos for some pathways.

 

Urinary Tract Infection (UTI) – by Teams Medical Practice

This pathway is designed to get the patient the right treatment in the quickest time possible and to place them with a General Practitioner at the exact point the appointment is needed. Working alongside Pharmacy First and processes in the practice, this clinical pathway utilises a redesigned process using AccuRx.

Hypertension

Two practices developed pathways for hypertension, taking different approaches and using different systems.

Hypertension –  by Saville Medical Group

This clinical pathway was developed to increase the uptake of annual hypertension reviews. Patients are sent a text and AccuBook which allows direct appointment booking and blood pressure can be taken remotely.

Hypertension -by St Albans Medical Group

This clinical pathway was designed to encourage patients to use the BP@Home app and submit home monitoring results.  Patients can loan a machine from the practice or use their own.

Oral contraception review

Three practices focused on contraception reviews using slightly different approaches based on their own processes and opportunities to make more efficiencies.

The following pathways have been designed to streamline the process for patients requesting their repeat oral contraceptive medication and who require a review. Practices have developed unique processes using the different processes and systems within their organisation.

Contraception review – by Glenpark Medical Group

This pathway aims to allow administration and nursing staff to gather all relevant information via either Accurx Florey or an interactive kiosk which will collect height, weight data and ask the same questions as and eConsult or Accurx. This information will then be placed with a GP/ANP for treatment options.

Contraception review – by Teams Medical Practice

This pathway aims to allow administration and nursing staff to gather all relevant information using Accurx Florey, before it is placed with a pharmacist for treatment options.

Contraception review – by Village Medical Group

The new process captures information on an eConsult form (sent via text message) which gathers patient information including weight and blood pressure. If a stable result is given, a 12 month prescription is issued.

New Patient Health Checks – by Village Medical Group

This pathway is designed to streamline the process for patients registering with the practice. The redesign involves the registration of new patient records, offering new patient checks to those with chronic or long term conditions only.

HRT – by Saville Medical Group

This pathway was developed with an aim to reduce consultation time for menopausal women wishing to start/consider HRT by using digital tools. The pathway uses AccuRx with a link to an eConsult form, which gathers information in advance to reduce appointment time.
