{"id":3728,"date":"2019-04-26T12:18:16","date_gmt":"2019-04-26T11:18:16","guid":{"rendered":"http:\/\/www.healthinnovationnenc.org.uk\/?page_id=3728"},"modified":"2021-10-20T12:50:52","modified_gmt":"2021-10-20T11:50:52","slug":"noac-alert-card","status":"publish","type":"page","link":"https:\/\/healthinnovationnenc.org.uk\/what-we-do\/improving-population-health\/cardiovascular-disease-prevention\/atrial-fibrillation\/atrial-fibrillation\/noac-alert-card\/","title":{"rendered":"NOAC Alert Card"},"content":{"rendered":"<h3><strong>Northern England Clinical Network (NECN) Non-Vitamin K Oral Anticoagulant medicines (NOAC) Alert Cards<\/strong><\/h3>\n<p><a class=\"btn btn-primary\" href=\"https:\/\/healthinnovationnenc.org.uk\/wp-content\/uploads\/2019\/04\/NOAC-Alert-Card-V3-order-specification.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">NOAC Alert Card Order Form<\/a><\/p>\n<p>Many patients are prescribed oral\u00a0anticoagulant medication to \u201cthin the blood\u201d and prevent strokes in the heart rhythm disorder atrial fibrillation and to prevent blood clots spreading in patients with clots in the veins of their legs (venous thromboembolism).\u00a0 Patients can be prescribed warfarin or Novel or Non-Vitamin K oral anticoagulant medicines &#8211; NOAC, also\u00a0more recently\u00a0known as Direct Oral Anticoagulant or DOAC. Patients receiving warfarin are issued with a patient held \u201cyellow book\u201d which is a record of blood monitoring and dosing regimen and also acts as an alert to healthcare professionals.\u00a0 However, there is no such standard and easily recognisable tool for the NOACs.\u00a0 NECN developed a standard patient alert card for patients treated with any of the NOACs\u00a0to clearly inform patients and healthcare professionals of\u00a0the key safety information that\u00a0they need and which, with the support of the Cardiac Rhythm Management subgroup of the North of England Clinical Network,\u00a0was developed with engagement of all the stakeholders in this clinical area.\u00a0This included input from the Network Stroke Advisory Group, Network Haematology Group, Network Pharmacy Group, Network Primary Care Group, Thrombosis Group, acute care physicians and patient representation groups.<\/p>\n<p>The card was designed to be easily recognisable with a colour familiar to current warfarin monitoring books (bright yellow). It\u00a0was launched in April 2015 across all secondary care NHS Trust pharmacies within the region (Scottish Borders to Lancashire and Yorkshire) and across all primary care and community pharmacies.\u00a0Electronic education and information was provided\u00a0for\u00a0all pharmacists, GPs and hospital physicians using the pharmacy networks and the medicines optimisation team infrastructure.\u00a0 An initial supply of cards was posted to all secondary care pharmacies (with advice to\u00a0distribute them across their trust sites) and to each individual community pharmacy in the region.\u00a0No supply of cards was issued to GP practices although they did all receive an example card from the AHSN when the AF Card Deck was distributed. All patients taking a NOAC should be provided with an alert card and additional supplies of these can be ordered by healthcare professionals as required.
