The Community of Practice (CoP) sessions are aimed at those working in Primary Care in the North East and North Cumbria.
Sessions include topics such as: risk stratification of patients with hypertension; involvement of the wider workforce optimise management of patients on their hypertension register; and running searches on EMIS and SystmOne to stratify hypertensive patients into high, medium and low priority.
