Death is a part of the natural lifespan and towards end of life, people’s needs around cardiovascular medicines can shift from prevention towards symptom control and comfort. This can be a difficult time to navigate for people, their carers and clinicians to communicate and co-ordinate care. Medicines, changes to them and support for their carers around medicines use provide a vital role to enable a good death. Join this webinar to learn how to put personalised care into action in your consultations to support people on this journey.

This webinar will be led jointly by Health Innovation North East and North Cumbria and Health Innovation Yorkshire and Humber and aimed at anyone with an interest in cardiovascular medicines, personalised healthcare, the ageing patient, multi-morbidities and polypharmacy.
