We are excited to announce our 28th EnCOP education webinar in our series linked to the Enhanced Care for Older People Competency Framework (EnCOP)

‘Developing and delivering a person-centred approach to proactive care for people with multiple long-term conditions and frailty’
This webinar will detail the learning from the NENC regional proactive care pilot which was delivered in 2 Primary Care Networks in North Cumbria and looked at the feasibility of implementing the national Proactive Care Guidance (Dec 2023).
We will share:
- The overall aims of the programme.
- How the teams identified the proactive care cohort
- How we developed the process of care based on the Year of Care approach to personalised care and support planning.
- How we supported workforce development and other enablers
- Key learning and recommendations
