How to Implement a Cholesterol Framework in Real World Primary Care

We need to restore routine care by prioritising patients at the highest risk of deterioration and cardiovascular events, with pathways that mobilise the wider workforce such as pharmacists, technicians and healthcare assistants, using digital tech to optimise remote care and self-care.

We will look at ways to work smarter, not harder, to reduce the General Practice workload, and hear from the experts who have designed frameworks and have first-hand experience of successfully putting them into practice in the real world.

This is the third in a series of webinars as part of the Tackling Cholesterol Together education programme, delivered in partnership with HEART UK, the AHSN Network and the NHS Accelerated Access Collaborative, led by expert clinicians and key national figures in cardiovascular disease.
