Safety, Systems and Support – Planning for PSIRF together

As the regions prepare for the implementation of the Patient Safety Incident Response Framework (PSIRF), we invited PSIRF implementation teams to join together to share learning from the first phase and our early adopters (Orientation); to understand the available data to inform local priorities (Diagnostic & Discovery), and support collaboration between ICBs and providers to develop effective systems to achieve improvement, cross-system learning responses, and sharing of insights and information to improve safety (Governance & Quality Monitoring)

The event was held across two sites; in Durham for the North East & North Cumbria ICB region, and Leeds for Yorkshire and Humber ICB region. We were joined by NHSE PSIRF leads, Tracey Herlihey (Durham) and Lauren Mosley (Leeds). We heard presentations from national and regional experts on PSIRF, understanding your local priorities and resources, interpreting data, the role of the ICB. Workshops were hosted specific to each ICB to look at working collaboratively on PSIRF implementation, understanding the governance and oversight roles, and to explore issues specific to maternity, mental health and independent providers.


The objectives of the event were:

  • To mobilise, coordinate and support networking across the ICS footprint
  • To build effective relationships with system leaders and attract key stakeholders, including nationally defined roles e.g. patient safety specialists, patient safety partners, MSOs and MDSOs
  • To support the development of a safety improvement plan
  • To support the delivery of 85% of Trusts completing activities relating to phases 1 and 2 by March 2023

This was a joint event hosted by the AHSNs and NHSE across the North East, North Cumbria, Yorkshire and Humber, bringing together PSIRF implementation teams from provider organisations and the ICBs to support a collaborative approach to PSIRF implementation and to develop region-wide systems for learning and improving safety.
