Polypharmacy: supporting the pharmacist workforce

Event Details

This event showcased the six projects funded through the AHSN NENC Polypharmacy Project Call and shared project outputs together with the resource toolkit developed on completion of each project.

The projects that received funding are as follows:

  • Reducing inappropriate prescribing in Primary Care, Getting Started
  • Polypharmacy Education
  • Supporting reductions in hypnotics prescribing
  • Reducing antibiotic prescribing through community pharmacy
  • Addressing inappropriate prescribing of intravenous antibiotics in futile clinical scenarios
  • Optimising the management of COPD exacerbations in primary care

The event was supported by the Great North Pharmacy Research Collaborative and is primarily aimed at Primary Care Network Pharmacists.

Wasim Baqir leads the NHS England Pharmacy Integration Fund Medicines Optimisation in Care Homes (#MOCH) Programme. He was the Lead Pharmacist for the Northumberland PACS Vanguard where he lead an integrated team of pharmacists and technicians, working collaboratively with doctors, nurses, social care and other professionals to develop and test person centred services for people across Northumberland (www.health.org.uk/pills).

NHS England and NHS ImprovementDr Bruce Warner works closely with the Chief Pharmaceutical Officer on the strategic and operational development of medicines policy for NHS England.

Prior to this post, Bruce worked as Deputy Director of Patient Safety at the NPSA and in NHS England where he led on the strategy for patient safety improvement and headed up the patient safety advice and guidance function.

Bruce has worked in most sectors of pharmacy including community, hospital, academia and at a PCG/T. He is also a visiting Professor of Pharmacy Practice at the University of Huddersfield.

Gemma Donovan is a Pharmacist Academic Practitioner working jointly as a Senior Lecturer n the School of Pharmacy at the University of Sunderland and as a Medicines Optimisation Pharmacist in NHS Sunderland Clinical Commissioning Group. Gemma’s work aims to explore how pharmacists can support medicines optimisation in all settings as well as working together across boundaries of care. This has included work on unlicensed medicines, medication adherence and medication safety. Gemma is also interested in how patients can be involved as co-creators in both research and practice innovation.

Rachel Berry

Rachel BerryAdvanced Medicines Optimisation Pharmacist

NHS County Durham CCGRachel Berry is a Medicines Optimisation Pharmacist working in the County Durham CCG MO team.  Within this role she focusses on a wide range of clinical areas, in particular clinical education, research and innovation, pain management and antibiotic stewardship.

Prior to working within primary care Rachel worked as a clinical pharmacist in multiple hospitals across the country including in Bradford, Sheffield, Stoke-on-Trent and Durham.  She has a diploma in Clinical Pharmacy, a certificate in Clinical Education and is an Independent Prescriber and therefore has a wide breadth of experience to draw on when it comes to polypharmacy and de-prescribing.

Alastair Paterson

Cumbria, Northumberland, Tyne & Wear NHS Foundation Trust (at time of project)Alastair is currently a locum pharmacist embarking on further postgraduate education. He was recently a locum pharmacist at Cumbria, Northumberland, Tyne and Wear NHS Foundation Trust. In his role here, whilst managing adult acute and rehabilitation psychiatric wards, he initiated a project to create educational interventions aimed at reducing inappropriate hypnotic prescribing. He developed and tested 3 educational interventions on inpatient psychiatric wards. The interventions were viewed favourably by ward staff and were linked to a reduction in hypnotic prescribing and administration rates.

As Managing Director/Community Pharmacist at Marton Pharmacy in Middlesbrough, Mike transformed the health and wellbeing of his community.

In 2009, Mike won the UK Community Pharmacist of the Year Award, and has since won national awards for Excellence in Training, Innovation and Excellence in Business, and an award from Mayor Ray Mallon for Services to the Community. Mike is an Honorary Lecturer at Newcastle University School of Pharmacy and has recently been instrumental in creating and launching the Award-Winning NHS111 Community Pharmacy Referral Service, which has evolved into the national NHS Community Pharmacist Consultation Service.

Joe is a Senior Clinical Pharmacist in Antimicrobials and a Clinical Teaching Fellow at Northumbria Healthcare NHS Trust. A Chief Pharmaceutical Officer’s Fellow in Global Health he is involved with work overseas in Tanzania around antimicrobial stewardship. We developed the Antibiotic Café as a novel training opportunity to address the growing issue of antimicrobial resistance focussing on antibiotic use in palliative care.

Lewis Sutherland

Lewis studied at Robert Gordon University in Aberdeen and did his pre-registration training in Edinburgh. From there he moved to Northumbria in September 2016 where he held several different roles and currently practices as a senior clinical pharmacist in rheumatology.

Lewis has an interest in osteoporosis and worked with the AHSN NENC on a primary care bone health project last year. This project aimed to identify and manage patients at risk of osteoporotic fracture across 16 GP practices in Northumberland who would potentially not have been identified for action by the usual reactive model of care.
